Referrals and Appointments
To see one of our doctors you need a referral from your GP or referring specialist.

Details required at the time of making an appointment include:

• Your name
• Your date of birth
• Your address
• Your phone number
• Name and provider number of the referring doctor

You need to bring to your appointment:

• Your referral letter
• All relevant investigations you have undergone
• Scans and x-rays

Please note: It is very important that you bring the pictures and not just the reports.
